MEMBER AGREEMENT /PRIVACY POLICY
STATEMENT The Go-Flex program will be operated by Priva Technologies (Priva). The Go-Flex program is strictly voluntary. Uses of the information are limited to identifying the individual and issuing a Go-Flex card. INFORMATION COLLECTED Applicants are asked to provide basic volunteered information for membership in the Program which consists of name, mailing and billing address, and credit card information collected at the start of the enrollment process submitted online. All such information is stored with the member records in our encrypted, secure database. Once you are a member of Go-Flex, you will be identified though a Go-Flex membership card. We respect your privacy and we never share or sell your personal information with any third parties. Any information collected through this site is intended to be used for this transaction only unless you opt-in for auto renewal. Your personal information is secured via SSL (Secure Socket Layer) Technology. The information you provide will be disclosed to Priva employees, contractors, and other agents who only need the information to assist in the procedures of the program. CONFIDENTIALITY All members' records, whether in hard copy or in computer systems are designated as private. An employee/contractor can only examine member records on a need-to-know basis to provide the service authorized. Each employee/contractor pledges to adhere to Confidentiality and Security procedures. PROCESS The applicant is assigned a unique Member ID after completing the online enrollment application. A receipt will be returned with a temporary membership card that can be used until your permanent card is processed and mailed. Your membership card will be processed and sent to you via USPS within 15 days of processing. REFUNDS You can cancel your membership with a 30 day notice and receive a pro-rated refund, less a $20 card processing fee. You will be refunded the remainder of your membership year beginning on the first of the following month. For example, if you cancel your membership on June 2nd, we will calculate your refund starting on July 1st. If you cancel your membership on June 20th, we will calculate your refund starting on July 1st.
